Report a Claim
Submitting a Longshore Act claim
Step 1
Before getting started, watch this video so that you know what to expect when you file a claim.
Then, complete the LS-202 Form (typed is preferred over handwritten) and save it to your computer.
Note: If you're an ALMA member with Origami access, click here to report your claim instead.
Step 2
Report the claim to AEU by sending us the LS-202 form. You have two options:
- Email it to claimopening@amequity.com.
- Fax it to 888.453.2562.
It is important to send claims immediately to AEU to prevent penalties against the member/employer for late reporting.
Step 3
We'll take it from here!
AEU electronically files the LS-202 with the DOL on the member’s behalf the business day following the establishment of a claim.

Submitting a State Act claim
Step 1
Complete the First Report of Injury Form (typed is preferred over handwritten). This form, which varies by state, can typically be found on each state's workers' compensation board website.
Step 2
Report the claim to AEU by sending us the First Report of Injury form. You have two options:
- Email it to claimopening@amequity.com.
- Fax it to 888.453.2562.
It is important to send claims immediately to AEU to prevent penalties against the member/employer for late reporting.
Step 3
We'll take it from here!
AEU will electronically file the First Report of Injury with the jurisdictional state on your behalf following the establishment of the claim.

Register for Online Claims Access
If this is the first time you are reporting a claim and you would like to be able to report claims online via our claims management platform (Origami), you will need to register for access by submitting the form at the bottom of this page. Please note that access is limited to one person per ALMA member and due to the sensitive nature of the data available in this system, we must first verify all requests before approval.
